Changes to Benefit Plans Effective January 1, 2012

New for 2012: Complete a Personal Wellness Assessment Survey* during Open Enrollment and save money on medical premiums.

  • $2,500 Value PPO Medical Plan

Our low-premium/high deductible medical plan will continue to include a $500 per family Health Reimbursement Account in 2012.

If you complete the Personal Wellness Assessment Survey* during Open Enrollment, you can earn an additional $300 per employee and $200 per spouse/domestic partner in your Health Reimbursement Account (HRA) for 2012. These amounts represent a $100 increase over the 2011 incentive.

Also in 2012, you and your family can earn an additional $200 ($100 spouse/domestic partner) for your HRA if you complete a Rollins Healthy Roads biometric screening early next year.

  • Monthly Tobacco Surcharge

The tobacco surcharge applies to any employee or covered dependent (age 18 or older) enrolled in a Rollins Medical Plan who has used a tobacco product in the last 12 months. This will increase from $30 to $35. Continue reading

Medical

Medical insurance is an important part of your financial security. After all, everyone wants to know that if something were to happen, you would have insurance there to help you pay for top-notch medical care for you and your family. Rollins is proud to be in a position to help provide that financial security for you and your family through our Medical Plan.

Rollins employees have three options when it comes to enrollment in medical benefits, provided through Anthem Blue Cross BlueShield:

  • PPO Plan (lower deductible)
  • PPO Plan (higher deductible)
  • Value PPO Plan with a Health Reimbursement Account (HRA).

A PPO (“Preferred Provider Organization”) is designed to offer you flexibility in choosing your medical providers within the covered network. Doctors, hospitals, and other medical specialists within the PPO network partner with your insurance to provide health care at reduced costs. You do not need to choose a “primary care doctor” or obtain a referral before you see a specialist.

Each of the plans has a:

  • Deductible — which is a set amount that you must pay towards your medical expenses before the Plan starts paying benefits.
  • Coinsurance amount — this is the percentage of your medical expenses that you pay after you have met the deductible
  • Out-of-pocket limit — this is a cap on the amount that you would have to pay in a year towards your medical expenses. Once that limit is reached, the rest of your expenses are covered at 100% for the rest of the year.

Rollins medical benefits also include 100% coverage on preventative care benefits – no deductibles or copayments – when you select an in-network provider.

If you enroll in the Value PPO Plan, Rollins will contribute money to a Health Reimbursement Account set up in your name. This money is available to help you pay part of your Medical Plan deductible. You receive part of your HRA contribution from Rollins automatically. You can also earn additional money from Rollins by participating in the Healthy Roads Wellness Program.

Dental

Regular dental care is important – both for overall health and oral health. Rollins employees are able to choose from four different dental plans available from Cigna:

  • Traditional
  • PPO
  • PPO Max
  • DHMO

Each provides coverage options for the main types of dental expenses:

  • Diagnostic/preventative (routine exams and cleanings, fluoride treatments, sealants and X-rays)
  • Basic (fillings and extractions)
  • Major (for example, crowns and dentures)
  • Orthodontia

Prescription Drugs

When you’re sick, going to the doctor is the first step to getting better. Sometimes you’ll also need to take medications to get you healthy again — and sometimes you’ll take those medications for a long time. That’s where the Prescription Drug Plan comes in.

Employees and their dependents enrolled in the Rollins Medical Plan are eligible for the Prescription Drug Plan offered through Express Scripts. You must use in-network retail pharmacies or the mail-order prescription service in order to receive prescription drug benefits.

Generally, the plan covers these expenses:

  • Up to a 30-day supply of medications purchased at in-network retail pharmacies.
  • Up to a 90-day supply of medications purchased through our mail-order pharmacy. The mail order pharmacy is great for maintenance medications — medications that you take regularly for long periods of time — like cholesterol-lowering drugs, insulin, and asthma medications.

Vision

Your eyesight is a vitally important component to your health. Healthy eyes and excellent vision are necessary for every facet of your life, from your career to your time with your family. Regular eye exams are also essential in helping detect eye disease that can take a drastic toll on your quality of life. As a Rollins employee you have access to vision benefits to help ensure your eyes are cared for and your sight maintained.

Vision benefits are provided to Rollins employees through the EyeMed Select Network Vision Plan. Your essential vision needs, such as eye exams and glasses or contacts, are covered under the plan. By selecting a provider who participates in the EyeMed Select Network, you will receive the highest level of benefits.

Disability Insurance

You work to provide for yourself and your family each day you’re on the job. But what happens if a sudden disablement makes you unable to do your job? This is where Rollins Disability Insurance is available to give you peace of mind.

Rollins employees have access to two types of disability insurance plans: Short-Term Disability and Long-Term Disability.

  • The Short-Term Disability (STD) benefits begin on the 7th day after an injury or illness and last for up to 12 weeks. Benefits will provide up to 50% of your weekly earnings (up to $1,500 weekly).
  • Long-Term Disability (LTD) benefits begin after 90 days of disability and cover up to 60% of basic monthly earnings (up to a $5,000 monthly maximum). LTD benefits can continue until retirement if necessary.

Life and Accident Insurance

Part of a healthy financial picture is life insurance. Whether you are single or have family members depending upon you, life insurance is important to ensure that loved ones are cared for and your financial obligations are satisfied after your death. That is why Rollins offers both Basic Life and Accidental Death and Dismemberment Insurance (AD&D) Insurance, as well as Voluntary Term Life (VTL).

Basic Life and AD&D Insurance

Basic Life Insurance pays a benefit of $20,000 to your beneficiary if you die while covered by the Plan. Accidental Death and Dismemberment Insurance provides a $20,000 benefit in the event of an accidental death or a partial benefit in the event of a serious injury. Both benefits are included in this policy, provided by Rollins at no charge to you.

Voluntary Term Life (VTL) and AD&D Insurance

You also have the option to purchase additional Life and AD&D Insurance for yourself, as well as for your eligible family members. In order to purchase life insurance for a child, you must purchase life insurance for yourself or for your spouse/domestic partner. Your benefit options as an employee are tied to your salary (such as 2x your annual salary), while benefit options for family members are flat dollar amounts.

Health Care and Dependent Care FSAs

Rollins Flexible Spending Accounts (FSAs) enable you to set aside tax-free money to reimburse yourself for routine expenses that you would have anyway. So why wouldn’t you want to save money on taxes at the same time?

Here’s how the FSAs work:

  • You choose an amount of money you want to contribute to each or both accounts.
  • The amount you choose is divided by the number of pay periods you have each year, and that amount is automatically taken out pre-tax each pay period.
  • You submit claims for reimbursement from your FSAs.

For a Health Care Spending Account, the money you set aside (up to $5,000 annually) can be used to reimburse yourself for eligible health care expenses not covered by another health care plan. Eligible expenses may include:

  • Medical, Dental, and Vision copays and deductibles
  • Prescription drugs
  • Over-the-counter medications, when you have a doctor’s prescription
  • Prescribed medical equipment
  • Laser eye surgery
  • Chiropractic expenses

A Dependent Care Spending Account enables you to put aside money for expenses to take care of your children or other eligible dependents while you and your spouse (if you’re married) can work and/or attend school full time. If your spouse is currently looking for work, you can use money you have set aside for day care expenses while your spouse is looking for work.

With this account, you are able to contribute:

  • up to $5,000 if single or married filing jointly, or
  • up to $2,500 if married filing separately.
